Request for Proposals
SOC 584 Mission: To establish a holistic opportunity center through which Southwest residents can move from low income to self-sufficiency.
RFP Purpose: SWAN began providing services at 584 Jefferson Avenue to create a bridge for individuals in recovery and re-entering the community from incarceration. The location historically associated with supporting residents lost substantial funding and – like other organizations that experienced cuts from units of local government – SWAN was forced to eliminate services. The ‘bridge’ collapsed. This was a blow to the agency and the community.
In an effort to restore opportunities for Southwest residents to receive these critical
supports in their own neighborhood, SWAN is seeking partnerships with local health and
human service providers who are willing to co-locate at 584 Jefferson Avenue, a minimum
of three hours per week. This request for proposals is, in and of itself, a unique opportunity
for providers to reach a population that might not otherwise access their programs and
services.
